COVID-19 Risk Self-Assessment and Disclosure forms

This a tool that takes a questionnaire in YAML and transforms it into a self-contained page with embedded JavaScript. The page transforms the answers in the questionnaire to a text-paragraph using pre-generated texts obtained using PHP-NLGen.
